He has an history of gastric reflux, as well, which was creating even more wear on his, already poor teeth.
After a thorough diagnosis and dental treatment planning, we have established a close relationship with the patients Gastroenterologist Doctor, in order to control the Gastric Acid Reflux in order to prevent more corrosion of his teeth and the treatment to be provided by us.

Now, after completing his treatment, he feels very confident and can't stop smiling.

Three months after placing her treatment she came back to have the final porcelain crown fitted. It was very difficult for him to chew and also he was very tired of having to deal with his dentures that were made in order to replace the missing teeth. The difficulty in chewing properly his food was creating an impact on his stomach as the food was taking longer to be digested, creating discomfort and increasing his acid reflux.
Because his teeth were missing for such a long time the amount of bone available was limited, mainly in the back of the upper jaw, where 4 dental implants had to be placed. Therefore bilateral sinus lifts were performed with the simultaneous placement of the dental implants. Eventually when his dental implants were healed (osseointegrated) and ready to be restored with the porcelain crowns, the patient reconsidered and asked Dr. Santos if he could also address the from teeth that were very worn, probably as a result of the increased acid reflux. These crowns did very well but unfortunately their life came to an end as they kept coming out as a result of decay on the root that was supporting the crowns. These teeth were extracted and immediate dental implants with fixed temporary crowns were installed. Once the implants healed completely (3 months after they were placed), two porcelain crowns were fitted, also allowing us to correct the poor position and look of the previous teeth that were too far forward and not straight, and also very discoloured..

These changes were tried even before anything was done to the teeth of the patient in order to make sure that the future look and bite would match exactly the patient expectations. Once we past this stage a minimal preparation of the teeth was done - just 0,3 mm of enamel was removed - making sure that the teeth would stay healthy, strong and unaffected, and a full set of upper and lower veneers and crowns was cemented. 
The final shape and colour of the crowns and veneers was decided by the patient, upon our recommendation, in order to mimic a look of a middle aged men's healthy teeth.

Following a careful diagnostic a treatment plan was presented to the patient. The plan was to follow the Teeth-in-a-day concept. This way, all her falling lower teeth were extracted and 6 dental implants were placed. Once the implants were placed a fixed temporary bridge was immediately fitted on top of the dental implants. This means that she was never without teeth during the all time that the treatment took place, she walked into the surgery with her falling fixed teeth and walked out with a fixed set of temporary teeth supported by dental implants that she wore until the final porcelain teeth were fitted. 
Once the implants healed, 3 months after they were placed, a final set or fixed porcelain teeth was fitted over the 6 dental implants. 
The patient is now able to bite comfortably, apples as well, and she can smile with confidence.

Sport injury leading to tooth loss.

This patients used to play rugby on a semi-professional level. As a result, he suffered a trauma on his upper right lateral incisor (UR2) that has lead the tooth to internal resorption and fraction.

This patient came to our practice referred by is own local general dentist. His upper front tooth had a failed root canal treatment with a large infection with indication to be extracted. Whistle the patient was debating about having a fixed 3 unit bridge done in that area, in order to replace the failing tooth or to have a dental implant, or a dental implant, we explained to him that in this situation giving the fact that the teeth next to the failing tooth were in a very good health it would not make any sense to sacrifice those teeth in order to replace the failing incisor. This way the patient decided to proceed with the dental implant. 
Although this case was very challenging because of the high smile line of the patient, and the fact that the failing tooth is in the front part of the mouth, we could achieve a very natural final result, restore the patient confidence without having to damage unnecessarily two perfectly healthy teeth.
1.  Tooth #21 (European class.), had a large periapical lesion. Tooth with an history of a failed RCT, external root resorption with indication for extraction.
2. Tooth extracted and a socket preservation procedure was performed. A temporary Maryland bridge was fitted in the area of the missing tooth.
3.  Soft tissue was manipulated from day 1 with the Maryland bridge.
4.  Implant placed atraumatically with guided surgery and a punch incision. While osseointegrating the Maryland bridge was kept in position.
5,6.  Once the implant was integrated the soft tissue started to be manipulated with a fixed temporary acrylic crown until correct emergence profile was achieved.
7,8.  Final restoration fitted. Zirconia abutment and Emax crown.

All his teeth had a very poor prognosis and an indication to be extracted as they were starting to affect his general health. After his teeth were extracted, we fitted straight away some temporary dentures and started to plan the next stages of his treatment. As a result of the Periodontal Disease, there was insufficient bone in order for him to have some dental implants placed. For that reason sinus lift and bone grafting had to be performed prior to implant placement, and then 4 implants were placed in the upper jaw and another 4 implants were placed in the lower jaw. Once the implants had integrated two implant supported over dentures were fabricated and fitted. Now the patient has his confidence restored, not only he has a perfect and natural set of teeth but he can also function normally being able to chew all kinds of food, steak, apples, etc... His teeth are to be kept in his mouth 24 hours a day, except when he need to clean them and also the implants that he can clip them out - this makes the maintenance of the over dentures very straight forward and quick.
